Stroke Program

Suffering a stroke can affect many aspects of an individual’s life, from social to mental to physical. Paramount Home Health Services offers a Stroke Management Program to provide patients with a multidisciplinary approach to address all aspects of life affected by the stroke. We utilize educational tools to help instill daily lifestyle practices in our patients, intended to prevent the possibility of secondary stroke.

Paramount Home Health Services Stroke Management Program aims to assist you or your loved one adapt to new life abilities after your stroke and reach a maximum level of independence. Our program includes:

• Evaluating, identifying and instructing on reducing risk factors for recurrent stroke

• Medication management

• Physical, Speech and Occupational Therapy

• Medical, physical and educational tools including blood pressure cuff, pill box, exercise pedaling device and educational booklet
